School Information

It is time for the 1st Installment for 2011-12 school year is, so please Download the School Verification Form. DO NOT PRINT DIRECTLY FROM THIS SITE. To obtain your list of Students, please login with your School ID and DOWNLOAD your list into an excel spreadsheet and attach it to the School Verification Form? REPORT 1.  These forms were mailed to each school as wells.  

As soon as we get your completed and signed form, checks will be issued within three to five days.  PLEASE REMEMBER, scholarships payments are disbursed in three installments:  1/3 in September, 1/3 in January and 1/3 in April.  In order for us to cut a check to the school, you must download, sign and return the Parent Verification Form each installment period.

In January 2011, Arizona School Choice Trust sent letters to all school informing them of a change in policy for issuing the scholarship checks for the 2011-12 school year.  After hearing from many of our schools and parents, we have eliminated that policy.  We are hear to help you and do not want to place additional burdens on our parents, so we will not be issuing checks to both the schools and the parents.  There will be no need for the parents to endorse the checks over to the school.  In order for the parents to verify and give authorization for us to cut checks to the schools for scholarship awards, we must receive a signed Parent Authorization Form from each parent.  This form was sent to each parents for their use and has been placed on-line as well..  The parent can come into your office and sign the form and you can send it back to us, OR the form can be sent home to the parents to sign and sent back to us directly from the parents, OR the parents can download this form from our website complete it, sign it and send back to us.
